This plan may include:
Supportive listening to determine and clarify needs
Understanding the main questions and concerns of this time
Advance Care Planning including defining what an advanced care plan is and its importance
Determining what treatments and care decisions the client wants
Resources to understand the term "life-sustaining treatments"
Reviewing existing challenges and needs as well as those anticipated in the future
Understanding present conditions and symptoms
Clarifying the supports already in place and those needed
Exploring what is important and what is most valued to make living the remainder of life meaningful and comforting
Clarifying hopes and ideas about end-of-life, and specific wishes before, during and after the dying process.
Documenting the above for the individual, and with their consent, sharing with those who will be supporting their end-of-life experience including their caregivers, family and loved ones.
Creating Legacy
Exploring what is important to leave behind for oneself and others, and determining how one wants to be remembered by creating keepsakes, conversations, and rituals for oneself and with those who are in one’s loving circle.

Support for Individuals & Loved Ones
Providing physical, emotional and spiritual support for individuals and families in hospital, Long-Term/Retirement Care Home, Hospice, or at home.
This may include:
Sitting vigil at the bedside
24/7 Availability to provide respite to loved ones
Comfort measures including supportive listening, mindfulness, visualizations, relaxation techniques, and Reiki.

Sustainable - $40 / hour

Educational Services
Providing reassurance and education to the person experiencing their end of life and for their family about the process and experience of dying.
Discuss Palliative Care/Hospice considerations
Provide support to assist families in understanding what is required for caring for a loved one during the last phase of their living journey
Grief Support

End-of-Life Preparation Course
A workshop facilitated in four separate two-hour sessions including handouts and opportunities to learn and practice skills shared in a safe and open environment.
I first met Lori at the Circle of Friends, a group run by the Cancer Assistance Program, where my wife Nancy was attending in 2003. She had a serious breast cancer and had joined the group and Lori was volunteering there at this time. They subsequently became connected and Lori began coming to our home on occasion. In late August 2004 when it was clear that Nancy was not going to recover, Lori attended to our home daily. Her calm and peaceful nature was a blessing for both myself and my daughters in assisting us through that terrible time. During that time, she showed as much compassion and understanding as if she were a family member but was able to act as a caregiver objectively. Her contagious smile was also a blessing at this time and helped as much as possible to ease our grief. She also provided good energy and she approached all of her tasks extremely professionally. My children and I would certainly recommend her to assist anyone in need.
